Universal Media Release Form

I acknowledge and consent that any image and/or audio recordings recorded as a participant in this Joni and Friends activity that contain my image and/or voice may be used without restriction by Joni and Friends for any purpose, including public broadcast.

Joni and Friends shall have unconditional ownership of and broadcast rights to such images and/or sound recordings. These conditions apply to any submitted media, such as video footage, audio recordings, images and still photographs provided by me as an on-camera participant for Joni and Friends use.

I hereby release and discharge Joni and Friends and its licensees from any claims of damage or
cause of action resulting from usage of these image(s) and/or audio recordings. I agree that the consent provided herein is irrevocable.

IF THE PARTICIPANT IS UNDER THE AGE OF 18, A PARENT OR LEGAL GUARDIAN MUST ALSO ACCEPT THIS THIS CONSENT / AGREEMENT AND STIPULATIONS ABOVE.
