As we shared in the television episode, health care choices - especially when you have a disability or a serious illness - can be very confusing. However, you have the right to give instructions about your own health care. You also have the right to name someone else to make health care decisions for you. This form lets you do both of these things. It is so important to discuss your decisions not only with your family physician with whom you share the same life values, but with your pastor and your family. Most critical of all, however, is that you talk about this document with your family attorney - he will help you with specific questions. Your attorney will want to keep a copy of your Will to Live in his files. You should also ask your doctor to keep a copy as part of your medical health record. We wish to thank our friends at National Right to Life for their pioneering work in developing the Will to Live Form in compliance with your state code. Be aware that it is possible that the statutes controlling this Will to Live Form may have changed since it was initially prepared - this is something you'll also want to bring to the attention of your family attorney. It's our prayer that God will give you wisdom in preparing your Will to Live. And be encouraged, for James 1:5 says, "If any of you lacks wisdom, he should ask God, who gives generously to all without finding fault, and it will be given to him."
